To analyze existing literature to identify similarities and differences in the epidemiological, clinical, diagnostic, and therapeutic aspects of irritable bowel syndrome (IBS) and conditions with IBS-like symptoms.
The review explores the similarities and differences among various conditions, including celiac disease, non-celiac gluten sensitivity, wheat allergy, increased non-gluten alpha-amylase/trypsin inhibitors, nickel allergic contact mucositis, consumption of high FODMAP foods, lactose intolerance, small intestinal bacterial overgrowth syndrome, inflammatory bowel diseases in remission, chronic pancreatitis, and gynecological disorders. Analysis of clinical studies has led to several hypotheses indicating that conditions with IBS-like symptoms share common features and pathogenesis with conditions with IBS-like symptoms, such as increased intestinal permeability, mucosal mediator release, upregulation of proinflammatory cytokines, neurogenic gut dysfunction, and disruption of the brain-gut axis.
It is currently postulated that using new diagnostic tools is advisable for identifying conditions with IBS-like symptoms in patients. The common pathogenesis suggests the potential use of a single therapeutic approach for patients with IBS and those with similar symptoms.
